    Photo I.D. for adults, for those 18 years or younger one form of the following: social security card, birth certificate, medical card, etc. Verification of address (i.e. utility bill, lease agreement, or piece of mail).

    Wednesday - Friday
    8:30am - 11:00am and 1:00pm - 3:00pm
